Available Services for Clients:

• Comprehensive Linkage and Advocacy: Assistance with housing resources, substance use treatment, social services benefits, homeless outreach providers, basic necessities items, and mental health services.
• Transportation Services: Provide transportation to any appointments to providers, probation check-ins, and medication pick-ups.
• Probation Collaboration: Update probation officers about client progress and provide individual service plan.
• Jail Discharge Services: include pre-release planning, transportation to probation from jail, ensuring that clients report to probation as instructed, and medication management.
• Court Process: The CSN Program plays a crucial role in the court process. We provide transportation to the courthouse when necessary and keep the court updated on any progress with the treatment plan, ensuring a smooth and efficient process.
• Housing and Homeless Support: CSN provides referrals to homeless outreach providers to ensure clients are placed in the Coordinated Entry System and Homeless Management Integrated System.
