About the Program

The CSN Program at Good Samaritan aims to support high-risk clients supervised by the Probation Department by linking them to essential services, offering transportation assistance, and empowering them to rebuild their lives. We strive to eliminate barriers, facilitate successful reintegration from jail to standard probation, and reduce reoffending risks through a warm handoff approach. Our goal is to provide continuous support, ensuring long-term stability and success for clients.

Available Services for Clients:

Comprehensive Linkage and Advocacy
Assistance with housing resources, substance use treatment, social services benefits, homeless outreach providers, basic necessities items, and mental health services.

Transportation Services
Provide transportation to any appointments to providers, probation check-ins, and medication pick-ups.

Probation Collaboration
Update probation officers about client progress and provide an individual service plan.

Jail Discharge Services
Include pre-release planning, transportation to probation from jail, ensuring that clients report to probation as instructed, and medication management.

Court Process
The CSN Program plays a crucial role in the court process. We provide transportation to the courthouse when necessary and keep the court updated on any progress with the treatment plan, ensuring a smooth and efficient process.

Housing and Homeless Support
CSN provides referrals to homeless outreach providers to ensure clients are placed in the Coordinated Entry System and Homeless Management Integrated System.
